Although the Care Data database uploads have been delayed until September 2014 you can already decide to opt out from this database.
Attached to this article are two versions of opt out forms you can print and fill out to pass onto your GP instructing them that you wish to opt out of the forthcoming care.data database, which will potentially be accessible by private non-NHS third parties.
Please note that after September 2014 it will be COMPULSORY to include your medical details on this database, so if you don't want this to happen you should start the opt-out process NOW!
As well as using the attached documents you could also custom write your own opt-out to your GP. Any of these options are acceptable and have to be taken seriously by your GP.

I meant to say you can custom write an opt-out LETTER to your GP, as this too is an acceptable (and legally binding) method.
