Children were prescribed the antipsychotic drug Risperdal for unapproved uses in children.
A CBS evening news report, exposed the shocking news about the use of Risperdal in children. Children who, in fact, were not psychotic at all, were prescribed the antipsychotic drug, Risperdal for unapproved uses in children. A law suit was announced on the makers of Risperdal for the increased use of diabetes and growth of breasts in boys as young as 4 years old. These boys have had to undergo surgery to remove the breast(s)

In addition to the horrifying news of what Risperdal does to children, there were increased warnings put on this drug for the use in elderly.
New Risperdal information has shown the increased incidence of stroke or stroke-like events in elderly Risperdal patients. In April 2003, Risperdal information was released by Johnson & Johnson saying letters would be sent to thousands of U.S. physicians advising them of the increased risk of stroke or stroke-like events such as blood clots and hemorrhages in elderly Risperdal patients.
WARNING: INCREASED MORTALITY IN ELDERLY PATIENTS WITH DEMENTIA
RELATED PSYCHOSIS Elderly patients with dementia-related psychosis treated with antipsychotic drugs are at an increased risk of death. Analyses of 17 placebo-controlled trials (modal duration of 10 weeks), largely in patients taking atypical antipsychotic drugs, revealed a risk of death in drug-treated patients of between 1.6 to 1.7 times the risk of death in placebo-treated patients. Over the course of a typical 10-week controlled trial, the rate of death in drug-treated patients was about 4.5%, compared to a rate of about 2.6% in the placebo group. Although the causes of death were varied, most of the deaths appeared to be either cardiovascular (e.g., heart failure, sudden death) or infectious (e.g., pneumonia) in nature. Observational studies suggest that, similar to atypical antipsychotic drugs, treatment with conventional antipsychotic drugs may increase mortality. The extent to which the findings of increased mortality in observational studies may be attributed to the antipsychotic drug as opposed to some characteristic(s) of the patients is not clear. "unapproved" uses of drugs

It is not uncommon that a drug devised for treating one disease (tested/approved) is discovered to have a very useful affect on some totally unrelated disease. This gets discovered when patients being treated for disease X (what the drug approved for) happen to also have disease Y and THAT shows improvement too.
First this gets reported and a study done indicated that yes, the drug ALSO has an effect on disease Y.
So doctors begin prescribing it immediately after THOSE studies published, especially if there aren't already drugs to treat Y or if those have much worse side effects. They do NOT wait until the drug company applies to do testing from the start (for use relative to disease Y) and in any case, if this is a drug already on the "generic" list, beyond drug patent period or too close to the end of that, no drug company is going to pay for getting the drug approved. .
You need an example. My wife has Lupus. That can be treated using drugs with dangerous side effects but MAYBE the anti-malarial hydroxychloroquine will suppress the lupus enough. That drug also can have side effects with long term continued uses but slowly developing ones so just test every six months. Possibly by now "approved for treating lupus" but do you imagine it started out that way?
BTW -- this is often a problem when the drugs are expensive and because "not on approved use" list the insurance company refuses to pay. Even though it might be well known that this is the only useful drug (or one much to be preferred to those on the approved list).
